As in "thanks ever so"? If I have that usage right, here in the states, it's used for its comic effect with a faux accent. I can't imagine anyone would use it seriously.

I think jtab has hit on something. The phrase "ever so" in US speak is probably mostly used unaffectedly in phrases like "ever so slightly." Most other uses would be a least slightly affected.
